  • You want ER test results, imaging reports, or discharge instructions
  • You need records from an emergency visit for another provider

  • You are a primary care patient at our Medical Clinic
  • You want clinic lab results or visit summaries
  • You need a prescription refill or want to message the clinic
Getting access Register yourself using the email address we have on file, or ask the front desk to send an invitation.

Seen in both places? You will have an account in each — they are separate systems and do not share logins. Not sure which you need? Call the ER on (602) 671-7990 or the clinic on (602) 671-7981 and we will point you to the right one.

Neither portal is for emergencies.

Messages are read during office hours only. If you need help now, call 911 or come to our Emergency Room, which is open 24 hours a day.

What you can do once you are in

Both portals give you your records securely, around the clock. The clinic portal additionally handles refills, messaging, and appointment requests.

See your results

Laboratory and imaging results as soon as they are released — no waiting for a phone call. Available in both portals.

Review your visits

Visit summaries and discharge instructions, so you can re-read what you were told when you were not at your best.

Request refills

Clinic portal. Some medications need a visit first before we can refill them safely.

Family access

Why are there two different portals?

Our Emergency Room and our Medical Clinic run on separate clinical record systems, so each has its own portal. ER records live in MyCareCorner; Medical Clinic records live in the Azalea Health portal. If you have been seen in both places you will have two accounts, and they do not share a login. We know this is not ideal, and our staff will happily point you to the right one if you call.

I was in the Emergency Room but my results are not in the clinic portal.

That is expected — ER results appear in MyCareCorner, not the clinic portal. Use the Emergency Room portal above. If you still cannot find them, call the ER and we will check that your invitation went to the right email or phone number.

How do I get an account?

For the Emergency Room portal, you are sent an invitation after your visit — call us if it did not arrive. For the Medical Clinic portal, you can register yourself using the email address we have on file, or ask the front desk to send you an invitation at your next appointment.

I have forgotten my password.

Use the “forgot password” link on the sign-in screen of whichever portal you are using. For security, our staff cannot see or reset your password over the phone — the reset link has to go to your own email.

My results are not showing yet.

Some results are released only after a clinician has reviewed them, which takes a little longer. If a result seems overdue, call the department that ordered it and we will check on it.

How quickly will someone answer a portal message?

Within one business day in most cases, and clinic messaging is not monitored overnight or at weekends. Anything urgent should be a phone call, and anything serious should be an Emergency Room visit.

Are the portals secure?

Yes. Both are dedicated, encrypted clinical systems, entirely separate from this website. We never ask for health information through website forms or ordinary email.

I need a billing statement, not a medical record.

Billing is handled separately from both portals. See our Billing & Insurance page, or call the billing team directly.

About your bill, before you worry about it

If something arrives from your insurance company after your visit, it is almost certainly not a bill. It is an Explanation of Benefits — a statement of what they paid. The large number on it is not what you owe.

For emergency care, federal law limits your share to the ordinary in-network copay, coinsurance and deductible your plan already sets. We do not bill you for the difference, and we never will.

If anything you receive is unclear, call us before you pay it. Our billing team would rather explain a statement than have you pay something you do not owe.
