Book an Appointment

Two ways to book a primary care visit — request one through the patient portal, or call the clinic and speak to our staff. Same-day appointments are usually available.

Do not request an appointment for an emergency.

For chest pain, stroke symptoms, trouble breathing, or serious injury, call 911 or walk into our Emergency Room now — open 24 hours, no appointment, and no callback needed.

Two ways to book

Pick whichever is easier

Call the clinic

The fastest way to be seen today — our staff can often find a slot that is not obvious from the outside.

(602) 671-7981

Have your insurance card and, if you are a new patient, a list of your medications to hand.

Request through the patient portal

Existing clinic patients can send an appointment request at any hour and our staff will confirm a time with you. Best for routine visits rather than same-day needs.

A request is not a confirmed appointment until we reply. If you need care today, call instead.

Walk-ins and same-day visits

If you wake up unwell, call us. We hold time for same-day sick visits, and speaking to a person is far more effective than waiting on a message. Our staff will tell you honestly whether the clinic can see you today or whether the Emergency Room is the better option.

Which service do I need?

Bringing your first visit

  • Photo ID and insurance card
  • Your current medications

A phone photo of the bottles is perfect.

  • Records from a previous provider

Helpful but not essential — we can request them later.

  • A few minutes early

New-patient paperwork takes about five minutes.

About your bill, before you worry about it

If something arrives from your insurance company after your visit, it is almost certainly not a bill. It is an Explanation of Benefits — a statement of what they paid. The large number on it is not what you owe.

For emergency care, federal law limits your share to the ordinary in-network copay, coinsurance and deductible your plan already sets. We do not bill you for the difference, and we never will.

If anything you receive is unclear, call us before you pay it. Our billing team would rather explain a statement than have you pay something you do not owe.
